How to make a word in dictionary dictated and the computer will display the meaning.?

It has something to do with voice command, I just do not know what are the processes to do to program something like it. Please help.

This is how it works: Point at any word, hold the Alt key and click: A nice pop-up will come up with the meaning of the word and, sometimes, a sound file too. 2. if you look an item up on a web [example www.dictionary.com] it will do this. However I do not know of any applications for home use that will do this.
